I (full name below) am the parent or legal guardian of the minor named above. I hereby give full consent and permission for my child’s name and likeness in photograph or video and/or voice on videotape or audiotape to be used by The United Methodist Church of the Resurrection and its assigns for promotional and educational purposes. This can include, but is not limited to, video presentation, audio presentation, website, broadcast and print advertising, book promotion and teaching curriculum. This consent applies to all nonprofit and commercial uses. I further waive any right that I may have to inspect and/or approve the use of my child’s likeness. It is acknowledged that I have waived compensation for all usage and that no other compensation is required.
